We are providers for most of the plans available in Central Illinois, including those listed below. If your plan is not listed, please call us for more information.

Payment Policy

• Health Insurance: We will bill your health insurance for you; however, you are expected to pay your co-pay at the time of service.
• Worker’s Compensation: If you have a valid Worker’s Compensation claim, we will bill the Work Comp Carrier. If the claim is denied, the responsibility then falls on the individual.
• Liability: We will bill liability insurance when there is liability coverage. When there is no coverage or if the claim is in dispute, the responsibility then falls on the individual.
• Self-Pay: We will accept cash, check, MC, Visa or Discover at the time of service.
